2024 Porter Hospital Addison County CHNA

Over the last year, C4BHI has partnered with the Vermont Department of Health (VDH) to collect information about the health needs of Vermonters.

This took the form of dozens of focus group and individual interviews with community members and subject matter experts in every county in the state. VDH has used this information to inform the development of the Vermont State Health Improvement Plan.

Porter Hospital in Addison County, VT recently concluded their CHNA and directly incorporated the C4BHI data into their report. Page 22 of the CHNA is a good example of this, as are the recommendations noted throughout the report. Overall, a very nice example of data sharing, collaborating across communities and service areas, and building a public health community across Vermont.
