Evaluation in progress with Adventist Health Community Well-Being Division

C4BHI is evaluating the effectiveness of a complex care pilot being implemented by the Adventist Health Community Well-being Division. Four communities in California are attempting to reduce healthcare utilization for vulnerable populations through coalition-building and the implementation of targeted, evidence-based interventions.Evaluation work involves aiding in the development of a comprehensive performance management system and collecting implementation, community engagement, and program sustainability and scalability data through repeated measurement with standardized and locally developed instruments.We are just beginning year 2 of a three-year evaluation cycle, and the community data dashboard for year 1 created by C4BHI staff allows project leaders and community partners to see progress in their work as it unfolds.
